Carr Hepburn offers a Change of Name Service for £115.00 (including VAT) This service is also available to people in receipt of Income Support or Income Based Jobseekers Allowance under Legal Aid. To change your name we require the following information:

• Form of Photo identification (Passport, Driving Licence) or Birth Certificate

• Proof of your address (2 utility bills or bank statements)

• Current Name

• New Name
